WELCOME BACK TO THE MOUNT
Dear Student-Athlete and Family,
We are so pleased you have decided to remain within the athletic family at Mount St. Mary's University. We hope you achieve and obtain new and rewarding challenges during this coming academic and athletic year.
Athletic Training continues to strive to provide our student-athletes with quality and efficient medical services. To achieve this, communication and cooperation between team physicians, athletic training staff, student-athlete and family is essential from the time of injury through rehabilitation and return to participation. Your cooperation in facilitating this communication is especially important. Listed below are important items to be completed prior to participation in any team related activities.
1.) (CLICK HERE) Please follow the instructions for completing the required ATS registration and forms. If you forgot your password, use the
forgot password feature. Please review/update as necessary the insurance information in the ATS system.
2.) (CLICK HERE) This section is for the student-athletes that are being treated with medication for ADHD/ADD. As an NCAA member institution, the
accepted treatments for this illness are banned substances. Complete proper documentation must be on file with Sports Medicine.
3.) (CLICK HERE) Please complete the Banned Substance Supplements Form with any supplements, over-the-counter medications and/or
prescriptions. IF NOT taking any medications please note N/A. You may mail, fax or email the form to the Sports Medicine staff
at firstname.lastname@example.org, fax: 301-447-6828, or upload the completed form in ATS. Upload instructions may be found in
the ATS registration packet.
Please complete all necessary information by July 15. Thank you for your attention and cooperation with Sports Medicine.
